community-mental-healthMental Health Support Services are trainings and support to enable individuals with significant functional limitations to achieve and maintain community stability and independence in the most appropriate, least restrictive environment. The treatment focus is on assisting the client with independent living skills and is therefore appropriate for older clients and adults.

The population served is young adults from ages 17 and up with mental, behavioral, emotional disturbances, and/ or substance use which have resulted in significant impairment and inability to self manage in the home or resident environment without supportive interventions. The client, if not treated, will be at risk of diminished self care, out-of –home placement due to unmanageable behavior, and/or repeated hospitalization. Clients referred for services may have a DSM IV diagnosis or may have documented mental health needs and/or substance abuse issues. Current referrals are made through the Department of Social Services, Mental Health, psychiatric institutions, court order, assisted living facilities, independent living facilities, and self referral.
